In an open differential, the spool allows one wheel to receive more power than the other as needed. In vehicles with very high mileage (typically 80,000 to 150,000 miles of severe service such as towing or frequent off-road use), spool wear can accumulate to the point where replacement offers better reliability and performance than continued patching.
