When in motion the transmission starts to slip in 1 and 2 gear then 4 and 5 gear jumps out of gear then goes in to limp mode

Vehicle makes clunking, jerking noises and bad clunking sounds when you put it into gear or shift gears, digital gear display shows vehicle is in 5th gear all the time, accelerates very slowly.

Well my car while driving it will completely shut off in me while im driving it. After it cranks back up it is very slow to take off
